Patients seeking admission to the Hospice Residence must meet the same admission criteria for hospice or palliative care services by Visiting Nurse and Hospice Care.
• Residential patients must have a diagnosis of a life-limiting illness with a anticipated prognosis of six months or less.
• Patients must agree to admission to the Hospice Residence, and the patient’s primary physician agrees to continue to manage the patient’s medical care.
• The hospice medical director may manage the patient's medical care if the patient's primary care physician is not available.
• If a bed is available, and the criteria have been met, every attempt will be made to admit the patient within twenty-four hours of the request.
